Thank you for your interest in Los Olivos Women’s Medical Group In order to schedule your appointment we will need the following information: The name of physician you would like to see, your name, date of birth, address, phone number, and your insurance carrier. Please note that the appointment time given is the time you are scheduled with the physician. Please check-in 30 minutes earlier so that we can complete your chart. You will be asked to complete a detailed medical history form, an address/insurance information form, and sign the Los Olivos credit policy and HIPAA notice of privacy forms. We will need all four forms completed as well as a picture ID and your insurance card on the day of your appointment. For faster service on the
day of your appointment, you may FAX the registration form to 356-8569 so
that your chart can be started. Please bring the original with you to the
appointment. It is important that you thoroughly complete the medical history
questionnaire; it will become part of your confidential medical record. If you
are a new obstetric patient, please also complete the obstetric questionnaire
and the genetics questionnaire. If you are an infertility patient, please
complete the infertility questionnaire.
