Appointments
If you are a new patient, please go to the new patient section.
If you are an established patient at Los Olivos and would like to schedule an appointment, please email or call for an appointment. In order to schedule your appointment we will need the following information: your name, date of birth, phone number, your physician, and the day/time you would like to be seen. The receptionist directory is available under staff directory. If you are scheduling an annual, you can download the history and complete it prior to your appointment: Annual form You can download the California State Informational Brochure on Gynecologic Cancers for review.
If you have new insurance or have moved, please update your Registration Information. For faster service on the day of your appointment, you may FAX the registration form to 356-8569 so your file can be updated. Please bring the original with you to the appointment.
If you are pregnant, please also complete the Prenatal Questionnaire and the Genetics Questionnaire. You will receive a copy of the Los Olivos Pregnancy Book at your first obstetric appointment. You can view the book in the obstetric information area of the website or download it prior to your appointment.
Infertility patients, please also complete the Infertility Questionnaire.
